Enter the street and house number of the last residence.

Enter the address where the family member intends to reside in Slovenia. This field is filled out when applying for the first permit.

The 12-digit reference number is issued by the photographer who uploads the photo to the electronic repository (efoto.gov.si). If you don't have the number, leave the field empty – you will bring the photo in person.

Enter the complete address: country, place, street, and house number.

Select the date when the family member entered Slovenia.

Enter the purpose of residence (e.g., family reunification, living with spouse, etc.).

Mark YES if the family member has a valid D visa for long-term stay.

Mark YES if the family member was an EU Blue Card holder in another member state.

Mark YES if the family member was a holder of an intra-company transfer permit.

Mark YES if the family member was a holder of a researcher permit.

Mark YES if the family member was a holder of a student permit.

Mark YES if the family member is of a person who had a researcher permit.

Mark YES if the family member's refugee status or subsidiary protection has ceased.

Mark YES if the family member has recognized subsidiary protection.

Select the family member's relationship to the applicant.

If you selected "Other", specify the relationship.

Select the main source of means of subsistence in Slovenia. If your source is not on the list, select "Other" and enter it in the field below.

Select the type of health insurance you have or will obtain in Slovenia.

If you selected "Other", describe the type of your health insurance here.

Select the type of travel document you use to prove your identity. Usually this is a regular passport. Diplomatic and official passports are issued to government officials.

Enter the passport number as stated in the document.

Select the date of passport issuance.

Select the country where the passport was issued.

Enter the place or authority that issued the document (e.g., Ljubljana, Moscow, Ministry of Interior RS).

Select the passport validity date. The passport must be valid for at least 3 months after the expected date of leaving Slovenia.
